  • Primary and secondary mental health care for children and adults, including specialist inpatient child and adolescent mental health provision, perinatal mental health, and forensic services with low and medium secure care.
  • Specialist mental health inpatient care for individuals with learning disabilities, alongside specialist community support for children and adults with learning disabilities and autism, including intensive support.
  • Extensive community physical health and well‑being services for children and adults, covering prevention, treatment, rehabilitation and long‑term condition management.

The Trust was first established in 2002 and employs approximately 7,500 staff who provide care from more than 400 sites. The organisation offers opportunities for medics, mental health, general nurses, children’s nurses, allied health professionals, psychology, administration and clerical staff, apprentices and volunteers, as well as those specialising in learning disabilities and community physical health services.

An opportunity has arisen for a Nurse, Social Worker or Occupational Therapist to join the Blackburn & Darwen Community Mental Health Team. You must be a motivated individual, committed to promoting recovery and well‑being for our service users as part of our multi‑disciplinary team. Excellent communication skills are essential for the role.

Blackburn with Darwen Community Mental Health Team are seeking to appoint a Band 6 Community Mental Health Practitioner. This post provides the opportunity for a forward‑thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care.

With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships. You must be a registered mental health practitioner, with excellent leadership and clinical skills.

You must have some post‑qualification experience in clinical work with service users experiencing a range of mental health problems.

You need to be able to work as part of a multidisciplinary team and have experience of planning, implementing and reviewing care needs.

You will also need to have a working knowledge of the care programme approach (CPA), Dialog+, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well‑being.

Main duties of the job

As a Band 6 MH practitioner within the team:

  • You will be regularly supported and supervised by the clinical lead/team leader, you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
  • You will be required to undertake the administration of medications and safe storage of same.
  • You will actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and Code of Conduct.
  • You are required to maintain links with your professional body and provide evidence for the revalidation process.
  • You will provide support to the nurse leadership within the multidisciplinary team and ensure the effective assessment, formulation, planning and monitoring of care given to service users.
  • You will exercise professional accountability and responsibility using skills, knowledge and expertise in changing environments, across clinical boundaries and in unfamiliar situations as outlined within the NMC Code of Professional Conduct.
  • You may be required to take on tasks that require you to operate electronic staffing record systems such as Appraisals/supervision.
  • You will maintain a positive and open attitude towards organisational change and service development.
  • You will work flexibly by covering the core community mental health team hours of Monday – Friday 9 am – 5 pm, to meet the needs of the service and the wider network.

Qualifications

  • Registered Mental Health nurse, Occupational Therapist or Social Worker
  • On‑going Professional Registration.

Knowledge

  • Understanding of health and safety policies and procedures, Knowledge and practice of Mental Health Act 1983.
  • Provide evidence of recent professional development.
  • Demonstrate understanding of how research has influenced practice.

Experience

  • Willingness to participate in mentor and preceptorship process in respect of own practice.
  • Experience of assessing, planning and implementing and reviewing care needs.
  • Able to work as part of multidisciplinary team.

Skills

  • Able to demonstrate and understand the concept of community care and the need to develop good networks.
  • Can demonstrate communication skills through practice and experience.

Other

  • Ability to travel around the organisational footprint to meet the essential requirements of the post.
