Role Overview

This is a fantastic opportunity for an experienced Social Worker or Team Manager to step into a pivotal leadership role within a Transfer of Care / Hospital Discharge Service. Hybrid working is available, up to two days per week from home.

This permanent role involves rapid onboarding and support to get you into the position quickly.

Key Responsibilities

• Oversee a busy service supporting safe and timely discharges across a 200-bed pathway.
• Allocate cases, monitor performance, and ensure assessments and reviews are completed within expected timeframes, including 14–28 day assessments and 6–12 week reviews.
• Guide and support the team through a light-touch triage system, ensuring efficient workflows and person‑centred outcomes.
• Maintain high standards of practice while supporting staff with decision‑making, prioritisation, and complex cases.
• Collaborate with a multidisciplinary network to drive effective discharge planning and support individuals to return home safely.

Qualifications & Requirements

As this is a statutory role, you must be a qualified Social Worker registered with Social Work England and have experience in supervising or managing within adult social care.
