Overview
This role will involve supporting people with mental health conditions in the new integrated mental health teams. The role provides professional nursing support to primary care network‑based teams, helping people engage with local communities and avail available supports.
Objectives
- Consider an individual’s needs holistically — mental and physical health as well as social needs
- Reduce the need for repeated long assessments and storytelling
- Provide early advice and guidance with signposting to relevant services, supporting individuals to use community resources fully
- Bridge gaps between community mental health care and primary care, acute care, social services and support
- Ensure easier access to the right level of care such as group programmes, therapy etc.
Responsibilities
- Provide holistic and specialist bio‑psycho‑social assessment for any patient experiencing mental health difficulties and offer specialist nursing assessment where required
- Provide advice to primary care to help them meet mental health needs within a primary care setting
- Carry out robust risk assessments, working collaboratively with the patient and their carer to provide a plan that will support them remaining safe
- Provide access to a broad range of specialist psychosocial interventions for individual patients where needed
- Provide expert advice regarding pharmacological treatment options available to patients, and seek clinical guidance from the designated Consultant Psychiatrist/GP for prescribing advice
Qualifications
Only those with Social Work England registration, and who can evidence at least 2 years of practice or experience after completion of ASYE.
