Eligibility & Checks

Key Responsibilities
- Work with adults to enhance independence and reduce the need for long‑term care.
- Proactively identify and address risks to prevent escalation.
- Demonstrate strong knowledge of support services; aid colleagues and citizens in accessing appropriate support.
- Collaborate with partner agencies including Greater Manchester Police, VCFSE sector, primary and secondary health‑care providers, children’s services.
- Embed “Our Manchester Behaviours” in all work.
Team Overview
The Adult Early Support Team operates virtually, delivering support without face‑to‑face contact, promoting independence and connecting individuals with local resources. The team is multidisciplinary and works fast‑paced.
