As part of our ongoing commitment to staff development and career progression within CAMHS, we are inviting applications for a role within the well-established CAMHS Transitions Team. This position provides an excellent opportunity for passionate, flexible, and enthusiastic professionals to join a creative service.
The CAMHS Tier 4 Transitions Team is looking to recruit a Young Person and Family Outreach Worker to work alongside Practitioners within the team to support young people in planning and actioning their discharge plan from an inpatient setting back into the community or next assessed location. The role is a PAL Dorset post and supports young people and families from Dorset. The role is both demanding and rewarding; a flexible, hardworking approach is essential to managing the demands of an inpatient setting while also being creative in the community space to support young people’s identified goals.
This post is offered on a part‑time basis covering a secondment opportunity within the team. The successful applicant will be expected to work a variety of shifts with flexibility in mind. The post holder will need to have experience of working therapeutically with children and young people with mental health presentations and their families. Experience of working in mental health services is essential.
Main duties of the job
Work with children and young people on the CAMHS Transitions Team caseload who require a creative, time‑limited intervention to achieve person‑centred goals to help young people’s ongoing journey back into the community.
As part of the wider multidisciplinary team, provide therapeutic interventions to children and young people with high risk and complex presentations. Support opportunities for engagement in community activities such as education, clubs, and time with family and friends.
Ensure the highest standards of service are provided, delivering and developing services to young people and their carers in accordance with Trust, Social and Healthcare departmental policies.
Must hold a full current UK driving licence and business insurance (access to the ward vehicle).
This is a fixed‑term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.
Person Specification
Job Specific Experience
- NVQ Level 3 in health or social care
- Experience working with young people
- Experience working with outside agencies
- Significant experience of working with young people with mental health problems and distress
- Ability to work in challenging and complex situations
- Ability to create innovative solutions working alongside patients to help meet their identified goals and needs
- Good understanding of risk and crisis management
- Experience of supporting young people with mental health problems
Information Technology
- IT skills, normally obtained through practice or practical training
- Experience of using Microsoft Office
- Knowledge of Data Protection Act
Additional Requirements
- Demonstrable skills in written and spoken English to a standard which enables the post holder to carry out the full range of duties and responsibilities of the role effectively.
- Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.
- A level of fitness and ability to participate and complete PMVA, Breakaway training and Manual Handling (practice and theory).
- Holds a full UK driving licence.
