Job overview
Working with your community nursing team, you’ll help deliver safe and person‑centred care that supports people to stay well at home.
You’ll be part of an integrated service that provides high‑quality nursing care to adults with a wide range of needs. Your work will help people manage their health, stay independent and feel supported in their own homes and residential settings.
You will join a friendly and committed team who work closely with colleagues across the Integrated Nursing and Conditions services. We provide a supportive environment where you can grow your clinical skills and contribute to continuous improvement.
Main duties of the job
What you’ll do:
- Carry out holistic assessments and develop clear, person‑centred care plans
- Deliver safe and effective nursing care in line with each person’s needs
- Work with colleagues and partners to support joined‑up community care
- Use and update the Electronic Patient Record (EPR) to maintain accurate notes
- Support people, families and carers to understand and manage health needs
What we’re looking for:
- A registered nurse with current NMC registration
- Good assessment, communication and clinical skills
- Ability to work well in a team and build positive relationships
- An interest in community nursing; you may be newly qualified or experienced
- A willingness to learn, develop and adapt to changing needs
Qualifications
- Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration
Knowledge
- Demonstrate an understanding of the role of a community nurse
- Knowledge of legislation impacting on service delivery
- Knowledge of the modernisation agenda
- Demonstrate an understanding of evidence based practice
- Knowledge of local initiatives
- Demonstrate an understanding of the MDT
- Basic IT skills
- Knowledge and skills in the assessment process
- Able to mentor students
- Evidence of professional development
- Have a working knowledge and understanding of First Visit Criteria
Experience
- Basic health and safety awareness
- Experience of working within a healthcare setting
- Experience of working with families and carers
- Experience of working in a team
- Experience of working with computerised recording systems
- Experience of multi-agency working
- Experience of working within a community setting
- To have recently worked within a nursing capacity
- To be able to provide education to patients, carers, staff and other health care professionals
