Support Facilitator (Home Care)
Staffordshire County Council Provider Services team is looking for dedicated Support Facilitators to deliver person‑centred reablement care in the community.
Shift times are 7:00 AM to 10:00 PM; part‑time positions with flexible shifts, including weekends. Early shifts start at 7 AM.
Applicants must live within 30 minutes of our base ST5 1BL and have a full UK driving licence and access to a vehicle for travel within the community.
Main Responsibilities
- Provide person‑centred reablement care in people’s homes, respecting dignity and promoting independence.
- Support individuals with daily living tasks, personal care, and medication administration.
- Communicate effectively with individuals, families and colleagues.
- Respond flexibly to changing needs and use digital tools to improve service delivery.
- Maintain accurate records and comply with health, safety and safeguarding standards.
- Undertake manual handling tasks as required.
- Drive across the local area using your vehicle.
- Complete interventions using assistive technology.
Qualifications & Requirements
- Experience in social care (NVQ2 or equivalent).
- Full UK driving licence and reliable vehicle.
- Living within 30 minutes of the base location.
- Physical ability to provide personal care & manual handling.
- Strong communication skills and positive outlook.
- Willingness to work flexibly across days, evenings, weekends and bank holidays.
- Full employment history from leaving secondary school (CQC requirement).
Additional Information
- Full paid training and ongoing development.
- Full shift pay, including travel time.
- Travel reimbursement at £0.45/mile, paid from Newcastle Library.
- Enhanced DBS check and pre‑employment health assessment required.
- This role is not eligible for sponsorship (effective 22 July 2025).
