Lymphoedema Community Nurse

We are currently seeking a Registered Nurse to join our Specialist Lymphoedema service. The successful applicant will work countywide, visiting patients in their own homes, supporting Community Nursing Services with the management of lymphoedema from an office base in Peterlee. The role involves assessing patients and providing specialist advice, signposting where appropriate. Successful applicants will be required to hold a first‑level nursing registration with the NMC; additional training specific to the role can be provided.
Main duties of the job
- Assess patients’ needs and develop, implement and evaluate programmes of care.
- Organise the delivery of lymphoedema care.
- Be aware of and communicate to team members changes to patient information and treatment plans, both verbally and in writing.
- Adhere to NMC standards of record keeping.
- Assist the Band 6/7 in setting, monitoring and maintaining standards of care.
- Participate in nursing audit and service improvement activities.
- Be aware of resource constraints and the importance of working within budget.
- Provide advice and act as a role model to other members of the team.
- Be proactive in managing risk and maintaining patient safety.
- Contribute to the day‑to‑day management of the Directorate, co‑operating with colleagues to provide an appropriate level of nursing service.
- Participate in education and training of other staff.
- Involve patients/carers in the planning and delivery of care.
- Participate in clinical supervision.
- Assist in resolving conflict and defusing situations where appropriate.
About us
We provide hospital services from two acute sites – Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester‑Le‑Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community‑based settings and providing care in patients’ homes.

Job responsibilities
The successful candidate will provide specialist lymphoedema care including assessment of lymphoedema, assessment and measuring for garments, reviewing plan of care. The role involves assessment of patient care needs, care planning, delivery of care and review. The successful candidate will be required to work independently as part of the wider Specialist team, reporting back to senior staff on duty.
Person Specification
Qualifications
- RGN with current NMC registration.
- Excellent standards of nursing care.
- Community experience.
- Lymphoedema training.
Special Requirement
- Driving posts must have a valid Full UK driving licence.
- Commitment, motivation and flexibility to work within a multi‑specialism service.
Statutory Registration
- Active 1st‑level NMC Registration adult with NMC valid registration UK NMC PIN.
- Demonstrate practical experience and understanding of lymphoedema.
- Demonstrate a keen interest in lymphoedema assessment, treatment plan and education.
- Flexible.
- Evidence of Lymphoedema practical skills.
- Evidence of a passion for the care of patients with Lymphoedema.
Experience
- Experience working as a Nurse/AHP.
- Experience working in the community.
- Nursing experience in a community setting.
- Experience in multiple areas.
- Experience of caring for patients with lymphoedema.
