Recovery Coordinator – Older Adults Team
Location: Kings Lynn Social Services.
Type: locum full‑time, approximately 3 months, Monday to Friday, 09:00‑17:00, rate £23 per hour Umbrella/LTD.

What you will do
- Provide high‑quality care coordination for older adults, developing and implementing recovery plans tailored to individual needs.
- Collaborate closely with multidisciplinary teams to ensure comprehensive support and effective communication between services.
- Conduct assessments to determine service users’ needs and recovery goals.
- Maintain detailed records and implementation reports to track progress and adjust care plans as necessary.
- Engage with families and carers to provide education and support, fostering a holistic environment for recovery.
