About The Role
As part of our expanding Adult Social Care workforce, we have three new permanent Assessment and Review Officer posts available across our community teams, including Learning Disability, Autism, and Hearing and Vision, and our two locality Adult Social Care Teams. Both full time and part time vacancies are available. We are proud and committed to working in partnership with customers and carers to enable them to lead more independent, healthy and fulfilling lives as part of their local communities.
We are looking for highly motivated individuals to work alongside Social Workers, using strengths based approaches, to help us deliver our statutory functions in a person‑centred, proportionate, and enabling way.
The Duties Of The Post Include
- Utilising a strength based model of practice and undertake assessments; developing and reviewing care and support plans to ensure they meet shared / best outcomes.
- Multi‑agency working in close partnership with customers, carers and other stakeholders.
- Supporting Social Work Teams in delivering their statutory duties under Care Act (2014).
The role will focus on our statutory Care Act 2014 review duties, carer assessments and supporting drop‑in sessions for our residents who may be in need of care and support.
The successful candidates can expect to be well‑supported in their role with supported opportunities to develop their practice. They will be provided with regular supervision and learning opportunities applicable to the role.
