PCN Clinical Pharmacist in General Practice
We are seeking an enthusiastic and motivated ClinicalPharmacist to join our Primary Care Network team.
This is an exciting opportunity to work in a patient-facingrole within general practice, supporting medicines optimisation, improvingpatient outcomes, and contributing to high-quality care.
You will work closely with GPs, nurses, and the widermultidisciplinary team, delivering clinical services and supporting safe andeffective prescribing.
Main duties of the job
- Undertake structured medication reviews and manage polypharmacy
- Support patients with long-term conditions
- Provide expert medicines advice to clinicians and patients
- Reconcile medicines following hospital discharge
- Contribute to medicines safety and clinical governance
- Support repeat prescribing systems
- Deliver education and training to practice staff

Job responsibilities
- Work alongside pharmacists, GPs, and wider PCN teams
- Be supported by a Senior Clinical Pharmacist and GP supervisor
- Lead on medicines optimisation, safety, and antimicrobial stewardship
- Deliver patient-facing clinics and medication reviews
- Be supported to develop into an Independent Prescriber

Person Specification
Qualifications
- Masters degree in Pharmacy MPharm or equivalent
- Postgraduate diploma or equivalent experience
Experience
- Strong clinical and therapeutic knowledge
- Understanding of general practice and primary care prescribing
- Excellent communication and interpersonal skills
- Ability to manage medicines optimisation issues
- Ability to work autonomously and within a team
- Experience in primary care
- Familiarity with SystemOne
- Motivational interviewing skills
- Ability to train and support others
