About the Role As an Intermediate Care Worker, you will play a key role in supporting older people to regain independence, improve wellbeing and achieve their personal outcomes. You will work as part of a fully integrated health and social care team, providing high‑quality, person‑centred care and support. We Are Looking For Someone Who: Has experience working as a Social Care Worker or in a similar care role Is passionate about improving outcomes for older people Demonstrates strong values, compassion and a person‑centred approach Understands Intermediate Care and its role in supporting recovery and independence Has excellent communication and organisational skills Enjoys working collaboratively as part of a committed multidisciplinary team Essential Requirements: SVQ Level 2 in Social Care with a commitment to complete level 3 within the condition of registration Experience delivering professional care and support to older adults Ability to work effectively within a busy and evolving care environment Why Join Us?
