Job Summary
The role of the Mental Health Well-Being Practitioner is to provide specialist nursing care to patients with mental health conditions or patients experiencing psychological distress. Deliver therapeutic interventions that reduce agitation and distress and promote recovery. Identify factors that increase and decrease agitation/distress and always deliver care according to the principles of least restriction.
Main duties of the job
The post holder will work with patients admitted to the Emergency Department and Inpatient wards as part of the multidisciplinary team. Responsibilities include assessment, development, implementation and evaluation of care to patients and families, delivering care planned by a clinician or registered nurse, ensuring timely patient and family interaction, contributing to efficient department operation, and working with minimal supervision.

Job responsibilities
- Therapeutically engage with patients who have a mental health condition or are experiencing psychological distress.
- Deliver care in accordance with the principles of least restriction and national guidance including NICE guidelines for managing disturbed behaviour.
- Understand continuous risk assessment of patients and escalated any increase in risk.
- Identify when patients are experiencing anxiety/distress and therapeutically engage to alleviate symptoms.
- Identify escalation of risk and actively de‑escalate the situation.
- Undertake therapeutic activities appropriate to each patient’s presentation.
- Undertake delegated clinical care duties to deliver agreed plans of care, acting on own initiative.
- Ensure patients receive high‑quality clinical care and good patient experience, respecting customs, religious beliefs and doctrines.
- Recognise and avoid situations that may be detrimental to the health and wellbeing of individuals.
- Advocate for patients within the department to ensure a patient‑orientated approach and meet NHS standards.
- Provide support and assistance to professionals in implementing plans of care.
- Collect specimens from patients, record routine observations and report abnormalities to a Registered Nurse.
Essential experience and qualifications
- Good level of general education including GCSE English and Maths.
- NVQ Level 4.
- Foundation Degree or Degree in a healthcare related subject.
- Minimum 6 months experience in an acute hospital setting or clinic.
- Previous experience working with patients with mental health conditions.
- Previous experience working with patients who present a risk to themselves or others.
- Previous experience as a healthcare assistant in a hospital or registered care setting.
- Experience taking clinical observations, customer care experience.
- Ability to document legibly and use electronic patient system when needed.
- Demonstrates awareness of resource management.
Desirable experience and qualifications
- Experience of therapeutic de‑escalation skills.
- Experience working within a team of mental health professionals.
- Previous experience in a health or social care setting, either statutory or non‑statutory service.
- Experience delivering psychological interventions.
- Nursing Associate on NMC Register.
- Knowledge of mental health act / mental capacity act.
- An awareness of the principles of social inclusion and recovery.
- Knowledge of therapeutic interventions to reduce risk of aggression, distress, self‑harm, anxiety, self‑neglect.
- Knowledge of mental health admission and discharge pathways.
- Understanding of patient confidentiality requirements.
- Ability to interact well with multidisciplinary team and patients/relatives.
- Punctual and reliable.
