Clinical Triage Nurse (24/7) Fast-Track Referrals

The role involves monitoring the Health Call system used by heart failure patients, escalating out-of-range observations, and supporting Palcall for people in their last 12 months of life. The post is part of a 24/7 service across the localities with potential cross-site work.
