Overview

This is a permanent position in the Chelmsford locality for Primary Care Mental Health service in Essex. The post will be based in General Practice, working with existing Primary Care teams to triage, assess, manage and follow up patients and provide Brief intervention to those registered with GPs in the area. You must be highly skilled and motivated, work autonomously and be confident in providing evidence-based psychological and non-psychological treatments for common mental health disorders following your assessments.

Other responsibilities include acting as a gatekeeper and guardian for referral to secondary services, working in collaboration with Primary Care colleagues, and providing support, specialist advice, consultation, education and problem-solving skills regarding assessment and management of service users with common mental health disorders. You will act as a signpost for internal and external services as required and be at the forefront of developing integrated care pathways.

Ability to work under pressure, manage stress, robust clinical judgement, autonomous working, and taking appropriate levels of responsibility are essential. You will participate in patient safety investigations, complaints, and relevant investigations in line with Trust policies. Mentorship and supervision of students, development of innovative practice to address service demands for referrals, allocations, KPI monitoring and clinical caseload management are expected. A UK driving licence is essential to drive to Primary Care network bases and trust sites. You will work with the GP, Primary Care team and Consultant Psychiatrists to ensure patients are supported in the most appropriate clinical environment from a multidisciplinary perspective.

Trio of core activities include triage and assessment of patients to ensure they receive the most appropriate support, management and delivery of safe services which support Clinical/Practice governance. You may undertake detailed physical health investigations such as ECGs and offer basic interpretation. Non-medical prescriber is preferred. Knowledge of Systmone patient documentation system is desirable.

Responsibilities

  • Triage and assess patients; facilitate access to the most appropriate support; ensure people are not passed around the system.
  • Manage and deliver effective and safe services supporting key elements of Clinical/Practice governance.
  • Work with GP, Primary Care team and Consultant Psychiatrists to ensure appropriate clinical practice environment from a multidisciplinary perspective.
  • Lead and participate in the development of integrated care pathways; provide educational support and training to enhance knowledge of Mental Health and Wellbeing within the Enhanced Primary Care team.
  • Provide professional clinical leadership at multidisciplinary meetings (referral/review, professional or safeguarding meetings).
  • Adhere to Trust Policies and Procedures; contribute to policy development and review; maintain professional/update knowledge.
  • Ensure professional accountability; liaise with the Trust Physical Health Lead; ensure clinical and professional supervision; disseminate best practice.
  • Identify and manage clinical and non-clinical risks; complete risk assessments; maintain Risk Registers.
  • Administer medications (nurse applicants), assess side effects, review concordance and clinical effectiveness; educate on medications, side effects and adherence; support physical healthcare understanding and training for staff and service users, carers and professionals as required.
  • Provide specialist assessment and intervention as required; lead formulation, delivery and review of evidence-based care following NICE guidance, including non-medication strategies.
  • Develop relationships with statutory and non-statutory agencies involved in Physical and Mental Healthcare; ensure collaboration with MDT and primary care teams to improve quality of services and safeguarding.
  • Deliver training for other professionals; complete timely statistics; monitor outcomes/KPIs; contribute to service improvements with the MDT and management teams.
  • Complete highly complex nursing needs assessments; ensure service performance meets KPIs and government initiatives; use computer systems (Word, Email, SystemOne) for communication.
  • Provide information and education to patients and relatives to support dignity, independence and quality of life.
