Community Mental Health Team in Central Norfolk

Working with people with moderate to severe mental illness who require specialist secondary care services as well as people with dementia and comorbid mental illness. The team provides a comprehensive service and works with a variety of other services to connect people to the support they need.

You will be based at Gateway House in Wymondham and work as part of a team assessing and treating people with moderate to severe mental illness who require specialist services and whose needs cannot be met elsewhere in the local system or by other partners. The team provides diagnoses and formulates holistic, person‑centred care and treatment. You may be required to undertake assessments in a patients own home based on clinical need and you need to be willing to travel across South Norfolk.

The post holder will assess people with moderate to severe mental illness. There will be some memory assessment work in the role. The postholder will develop treatment plans with support from an experienced team that includes Consultant Psychiatrists, Clinical Psychologists, Senior Nurses and an Occupational Therapist. The postholder will ensure service users have a clear recovery and discharge plan from the beginning of treatment.
