• Salary: GBP 39,959 – GBP 48,117 per annum (Pay Scale Band 6)

Job Structure

• Permanent
• 37.5 hours per week
• Day shifts (Monday to Friday)

Key Responsibilities

• Deliver compassionate care and set a strong example for colleagues.
• Lead advanced care planning with patients and families.
• Work closely with the wider PCN team to improve patient outcomes.
• Develop and implement best practice initiatives.
• Coordinate patient activity and ensure high standards of care.
• Ensure effective communication with patients, families, and the wider community.
• Participate in clinical governance, audit, and quality improvement activities.

Qualifications

• Experience in specialist palliative care, including complex needs and end‑of‑life care.
• Experience in advanced care planning and advanced care discussions.
• Registered nurse on the Nursing and Midwifery Council (adult registration) with adherence to professional standards.
• Ability to connect with patients and families emotionally.
• Demonstrated research, data collection, and case study development contributions.
• Commitment to full‑time hours Monday through Friday.
