Support Worker – Glassmoon Services

We are looking for a caring and dedicated Support Worker to provide 24‑hour support to young people with autism and/or learning disabilities. The role involves enabling them to live as independently as possible, build community connections, and pursue personal goals such as education, employment, and leisure activities.

Responsibilities

  • Deliver person‑centred care that aligns with the individual’s support plan and NHS standards.
  • Engage with clients to help them pursue their interests, maintain skills, and access tailored support.
  • Support the development of relationships and community connections for each person.
  • Maintain accurate support plans and other records to ensure effective service delivery.
  • Communicate any changes in a client’s needs to the team leader or senior support worker.
  • Safeguard the well‑being and safety of all individuals in care.
  • Assist clients in keeping relationships with family, friends, and other important people.

Requirements

  • Drivers preferred – all checks (CRB, ID, address, references) are funded by Glassmoon Services Ltd.
  • Right to work in the UK.
  • Satisfactory criminal record checks.
  • References.
