INTERNAL Community Development practitioner – SCPHN (HV or School Nurse)

We are looking for a proactive, innovative and autonomous practitioner, who is passionate about delivering the public health agenda in a creative manner. The Families health & wellbeing 0-19 service is committed to improving the health, wellbeing and providing opportunities for children and families in our community. We work closely with our service users and families to create inclusive and supportive initiatives allowing families to easily access our services via a variety of platforms; including digitally through podcasts, virtual groups and social media platforms.

Main duties of the job

In this role you will need to be a confident practitioner who can work well independently, as well as part of the team. You will require excellent IT, communication and relationship building skills, enabling you to work within our community in a variety of settings, facilitating groups, workshops and community events.

As a community development practitioner, you will work in collaboration with the 0-19 team and local partners to:

  • Work autonomously, in line with your scope of practice, competence and policies.
  • Engage with local family hubs, service users and partners to identify community strengths, needs, and priorities.
  • Develop and support community-led initiatives and projects.
  • Build strong relationships with local organisations, partners, and stakeholders.
  • Facilitate groups, workshops, and community events.
  • Support individuals and groups to access services, resources, and opportunities.
  • Monitor and evaluate community programmes and outcomes.
  • Promote the service to families and community partners.
  • Complete quarterly and annual reports demonstrating the effectiveness of initiatives that have been implemented.

Qualifications

  • Specialist Community Public Health Nurse – Health Visitor or School Nurse.
  • Registered Nurse or Midwife.

Experience & Knowledge

  • Evidence of experience or knowledge of Community Development role.
  • Evidence of Multi-agency working.
