Prison-to-Community Support Case Worker

myGwork – LGBTQ+ Business Community is seeking a dedicated Case Worker to support individuals transitioning back into society from HMP Durham. You will work with participants in their last 12 weeks of sentences, providing guidance, goal setting, and connecting them with community resources to aid their rehabilitation.
