About the Role:We are recruiting a dedicated Outreach Family Support Worker to provide high-quality, home-based Early Help support to children and families using a whole-family approach. This temporary long-term role involves working with children and young people aged 025* and their families, helping to prevent the need for statutory intervention and ensuring access to universal services.

You will take whole-case responsibility for complex families, coordinating support, completing Early Help Assessments, and acting as Lead Professional when required. The role includes providing intensive and targeted support, facilitating family engagement, and supporting children and young people with emotional and mental health challenges.

Key Responsibilities:Case management for complex families stepping down from social careConduct Early Help Assessments, coordinate Team Around the Family (TAF) meetings, and monitor progressSupport children and young people with high-risk or complex needsDeliver practical parenting strategies and de-escalation techniquesMaintain accurate records on electronic systemsAttend supervision, planning meetings, and professional reviewsPromote safeguarding, anti-discriminatory practice, and equality of opportunitySupport supervised contact sessions where required

Requirements:Ideally Car driver with access to own vehicleRelevant Level 3 qualification in Health, Social Care, or Early Help (Level 5 preferred)Experience working with children, young xwzovoh people, and families in a support or social care roleKnowledge of safeguarding, risk assessment, and family engagement methodsSolution-focused, family-centred approach (e.g., Signs of Safety)Ability to work independently and as part of a multidisciplinary teamExcellent communication, record-keeping, and organisational skillsEnhanced DBS required
