Job Overview

We are looking for a Health Care Support Worker for the Discharge Lounge at Royal United Hospital Bath. The Discharge Lounge supports patient flow by providing a safe, calm environment for patients awaiting transport home or to a Community Hospital.

Responsibilities

  • Patient care and clinical support, including enhanced personal care, delegated clinical tasks, recognise and escalation of deterioration, support safe mobility, and assist with nutrition and hydration.
  • Discharge coordination and flow: collection of patients from wards, support TTA readiness, update patient flow systems, liaise with transport services, communicate with wards and MDT, apply exclusion criteria, and assist porters with transport.
  • Documentation and information handling: complete delegated documentation, ensure accuracy and confidentiality, and support Datix reporting.
  • Environment, equipment, and safety: maintain a safe clean environment, perform equipment checks, support manual handling safety, and assist in emergency situations.
  • Communication and patient experience: provide compassionate communication, support patients with cognitive impairment, and manage expectations professionally.
  • Professional development and service improvement: complete mandatory training, support induction of new staff, contribute to QI initiatives, and participate in supervision and appraisal.

Qualifications

Essential criteria

  • NVQ Level 2 in Health & Social Care (or equivalent experience)
  • Years of experience
  • IT skills

Desirable criteria

  • Evidence of work experience in a Discharge Lounge
  • Care Certificate or willingness to complete
  • Evidence of work experience

Essential Knowledge & Skills

  • Effective communication skills
  • Ability to work under pressure

Desirable Knowledge & Skills

  • Relevant training
  • Very good communication skills
  • Evidence of IT training
