We’re looking for a passionate and proactive Health and Wellbeing Practitioner to join our team at HMP Cardiff on a full-time basis (37.5 hours per week). This is an opportunity to support individuals on their journey towards positive change, helping them improve their health, wellbeing, and prospects for a successful return to the community.

You’ll manage a varied caseload, deliver tailored interventions, and work collaboratively with service users to develop meaningful care and release plans that support long-term recovery and wellbeing.

What You’ll Be Doing

  • Managing a caseload of service users and coordinating their treatment journeys
  • Conducting assessments and developing person-centred care plans
  • Delivering structured 1:1 interventions and facilitating group sessions
  • Providing harm reduction advice and wellbeing support
  • Supporting release planning and through-care arrangements for individuals returning to the community
  • Carrying out second signatory duties and supporting clinical services
  • Working closely with multidisciplinary teams and partner agencies
  • Maintaining accurate case records and performance data to meet service targets

What We’re Looking For

  • Experience supporting individuals with complex health, wellbeing, substance use, or related needs
  • Strong assessment, case management, and communication skills
  • Ability to build positive relationships and motivate change
  • Excellent organisational skills and attention to detail
  • Confidence working in a fast-paced, target-driven environment
  • A collaborative and solution-focused approach

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
