Job Summary
Community Homeless Health Support Worker – a permanent, full‑time role based in Montpelier, Bristol, serving Bristol, North Somerset and South Gloucestershire. The closing date for applications is 16 June 2026. Please note that a Certificate of Sponsorship is not available for this position; if you need a visa to work in the UK you must have at least 14 months remaining until expiry and you must disclose your expiry date within your application.
Key Responsibilities
- Carrying out outreach plans as prescribed by registered therapists, working independently within agreed protocols.
- Assisting clinical staff with assessment and treatment of patients discharged from hospital.
- Following a prescribed care plan to support patients at home, including arranging GP surgeries, outpatient appointments and general health care.
- Promoting healthy and independent living practices for patients.
- Collaborating with other organisations across health and social care sectors (e.g. St Mungo’s).
- Performing general administrative and non‑clinical tasks such as photocopying, filing, liaison and telephone calls.
Qualifications
- Essential: Maths and English GCSE or equivalent.
- Desirable: Previous experience as a support worker.
- Desirable: NVQ Level 3 in Direct Care.
Skills, Experience & Knowledge
- Ability to work independently without direct supervision.
- Strong communication skills – electronic, written and verbal.
- Experience of working with adults in a health and social care setting.
- IT proficiency (email, Word, Excel).
- Ability to prioritise tasks effectively.
Disclosure and Barring Service Check
This role requires a Disclosure and Barring Service check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
