Overview

An exciting opportunity has arisen for an experienced Registered Skilled Worker (RSW) to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS) in the Cupar Locality. The Intermediate Care Service provides rehabilitation to the adult population within the locality. This will be delivered to service users as in-patients, out-patients or within their own home. The primary function of this role is to work with the Occupational Therapy, Physiotherapy & District Nursing Services within Intermediate Care to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital.

Responsibilities

  • Work with Occupational Therapists, Physiotherapists & District Nursing to deliver therapeutic treatment programmes to service users in the ward setting or in their own homes.
  • Carry out ongoing treatment programmes for a mixed caseload of mainly older people.
  • Provide daily care in the community, a service provided 365 days of the year from 0800–2200.
  • Work as part of a supportive multidisciplinary team and, when required, work in other community hospital sites.
  • Travel throughout the area as this is a community post; flexible 24.7 hours over 7 days.
  • Receive regular supervision and participate in Personal Development Planning (PDP) and training opportunities.

Qualifications

  • SVQ Level III in Health and Social Care or equivalent.
  • Experience working as a support worker is desirable.
  • Experience working with older people is desirable, as they are the main group of service users referred.
  • Good organisational and excellent communication skills; ability to work independently as well as within the team.

Additional Information

  • The ability to travel throughout the area is essential as this is a community post.
  • Our service offers regular supervision, PDP and supports training opportunities.
  • There may be occasions when you will be required to work in one of the other community hospital sites.
