Experienced Social Worker – Transfer Of Care Hub

This 6‑month fixed‑term role, with the possibility of extension up to 2 years, is offered at Wirral Council.

About the Role

Located across Arrowe Park Hospital and Clatterbridge offices, you will work within a dynamic multi‑disciplinary team supporting safe, timely discharges from hospital while promoting independence, wellbeing, and positive outcomes.

Responsibilities

  • Deliver high‑quality health and social care services for adults and carers, ensuring practice reflects the Care Act 2014, strength‑based working, personalisation, and social inclusion.
  • Work with adults with physical health needs, mental health needs, older adults and carers, often those experiencing complex circumstances.
  • Support the development of others, including ASYE/newly qualified social workers and Care Navigators, modelling good practice and championing professional standards.

Qualifications and Requirements

  • Strong background working with adults with complex needs, carers and families.
  • Skilled in assessments, support planning, safeguarding and working within statutory frameworks.
  • Excellent written and verbal communication skills, with the ability to manage complex and sensitive situations, prioritise demanding workloads and maintain high‑quality practice.
  • Social Work qualification required (e.g., Degree in Social Work, DipSW or equivalent) and registered with Social Work England.
  • Will undergo a DBS check.
  • Right to work in the UK or meet visa sponsorship criteria for a Skilled Worker visa.
