Off the Record (Bristol) is seeking a dedicated Youth Transition Worker to join their Mental Health and Wellbeing Integrated Network Team in South Gloucestershire. The role focuses on supporting young people transitioning from Child and Adolescent Mental Health Services (CAMHS) while ensuring they have the required support during their transition.
The successful candidate will manage a caseload, build therapeutic relationships, and deliver interventions to help young people from diverse backgrounds. This permanent role offers salary pro-rata starting at £13,441 based on 0.5 FTE hours.
