Community Heart Failure Specialist Nurse

This is an exciting opportunity for a highly motivated and enthusiastic nurse to join our community-based team of Heart Failure Specialist Nurses, delivering advanced practice care to patients with chronic heart failure in the Harrogate and Rural District Area.
At Band 7 the post holder will have at least 2 years’ experience managing chronic heart failure patients and must have completed a Heart Failure module. They will also be an Independent Prescriber.
We welcome applicants willing to undertake these courses and gain experience, starting in a developmental Band 6 HFSN role with a clear competency framework to progress to Band 7.
Main Duties of the Job
The post holder will manage or work towards managing an identified group of patients with complex health and social care needs, using a case-management model. They will develop highly specialist nursing knowledge and advanced skills to proactively manage patients with Heart Failure, improving the quality, experience and choice of care, enabling patients to remain at home longer.

Qualifications
- NMC registration
- 2 years’ experience at Band 6 caring for HF patients
Heart Failure Course
- Pass Heart Failure course
- Willing to undertake the course in a developmental role

Compensation
£39,959 to £56,515 per annum (pro rata).
