Job Summary

As a Community Support Worker (CSW) you will work within the Intensive Community Care pathway and Urgent Community Response team covering Oxfordshire county. You will visit people at home who are experiencing an urgent health and social care crisis, helping them recover and stay in their own homes where safe.

Responsibilities

  • Deliver hands‑on care, planned advice, support, and instruction to patients and, where relevant, their carers and other professionals, ensuring consistent care in line with treatment plans.
  • Communicate effectively with patients, including those with mental health needs, and their families/carers using verbal and non‑verbal techniques to progress agreed management plans.
  • Evaluate emergency situations and act according to established procedures and protocols, including knowledge of DNACPR protocols.
  • Carry out duties across community settings, including lone working and visiting service users in their own homes, travelling independently between sites as required.
  • Work independently and under the direction and support of a registered clinical practitioner.
  • Operate within shift patterns and occasional weekends between 8 a.m. and 8 p.m., 365 days a year, and be flexible to accommodate service demands.
  • Travel across the county as needed, working within a designated locality area but participating in county‑wide services.

Qualifications & Experience

  • Essential
  • Level 2 or 3 Diploma in Health and Social Care.
  • Evidence of self‑directed learning.
  • Full and valid driving licence with access to a vehicle for work.
  • Previous experience as a care worker/formal carer with adults.
  • Experience in phlebotomy and taking observations.
  • Experience of working autonomously within a community setting without direct supervision.
  • Well‑developed communication and interpersonal skills.
  • Ability to work as a member of a team.
  • Ability to recognise stress or anxieties in colleagues, patients, and families and offer appropriate support.
  • Ability to work flexibly to meet service needs and be open to change.
  • Awareness and acceptance of the rights and needs of service users and staff.
  • IT literacy with good keyboard skills and effective time‑management to prioritize workload.
  • Competent in clinical tasks related to the role.
  • Desirable
  • Advanced qualification beyond Level 3.
  • Experience working within a community health care team.
  • Experience of receiving clinical supervision.
