Band 7 Lead Nurse – Transition to Adulthood.
As the first postholder in this role, you will have the opportunity to design, influence and embed a high‑quality transition pathway that supports young people as they move from children’s to adult services. Working across multiple specialties and professional teams, you will ensure young people receive seamless, safe and person‑centred care throughout their journey.
Main duties of the job
- Lead the development and implementation of the Trust’s transition strategy.
- Champion the needs and voices of young people and their families.
- Work collaboratively with paediatric, adult and community services to improve patient experience and outcomes.
- Drive quality improvement, innovation and service transformation.
- Provide expert leadership, education and support to clinical teams.
- Develop strong partnerships with internal and external stakeholders to enhance transition pathways.
Job responsibilities
- Registered Children’s Nurse with 2 years experience at Band 6 or Band 7.
- Teaching / Mentorship qualification.
- Evidence of ongoing professional development.
Experience
- Experience in multi‑disciplinary staff working.
- Experience of working independently.
- Experience in education of staff.
- Experience of working with young people who have long‑term medical conditions.
