An opportunity has arisen within the Derbyshire Dales Mental Health Adult Care Team for a Social Care Practitioner.
The team works with people with mental health difficulties from the age of 18. You will need good working knowledge of the Care Act 2014 and the Mental Capacity Act 2005, as you will be assessing clients’ care needs and working with them and their support networks to create support plans. You will also need knowledge of the Mental Health Act and other relevant legislation. Using person‑centred, strengths‑based approaches you will support people to live independently or in a more formal support setting.
A good working knowledge of Safeguarding is also essential.
We continue to offer a hybrid approach to working, with the team working from home and from the office based on the needs of the service.
