Responsibilities

The post holder works within a multi‑disciplinary 24/7 Crisis Resolution and Home Treatment Team, providing urgent assessment and support to people experiencing acute mental health crises, both functional and organic. Under senior supervision, the role includes supporting mental health and risk assessments, delivering home treatment, providing evidence‑based interventions, monitoring care and treatment plans, contributing to reviews, and supporting care packages, including for organic presentations. The post holder will supervise junior staff and assist with the operational duty system on a rotational basis, contributing to gatekeeping, early discharge, out‑of‑hours access, and crisis assessments. They must follow organisational systems and processes, manage competing priorities, and act as a clinical resource for junior colleagues. The role also supports agile working and may include delegated responsibilities as appropriate.

The post holder communicates effectively with service users, carers, colleagues and external agencies, using tact, diplomacy and age‑appropriate approaches. They provide clear written and verbal information, lead reviews, advocate for service users and follow Trust communication policies. They work within their professional code, participate in supervision, and provide supervision to junior staff. They plan and organise their workload, manage complex activities, adapt to changing demands and apply legal frameworks such as the Mental Health Act and Mental Capacity Act. They use IT systems safely, maintain required training and undertake safety intervention and disengagement where needed.

  • Assess needs, plan and review care, deliver interventions, safeguard, support physical health, enable meaningful activities and work within the framework.
  • Ensure confidentiality, promote equality, and reduce risks.
  • Manage resources responsibly, support staff development, mentor learners, undertake appraisals and delegate tasks appropriately.
  • Maintain accurate records, follow information governance standards and support research and audit.

The role involves working across varied settings, managing interruptions, handling distressing situations, and adhering to infection control, health and safety and environmental policies. Agile working, computer use, driving and exposure to challenging conditions are required.
