Description

• Full Time PLVE Dentist required for 4-5 days per week in a busy, mixed, well established, INDEPENDENTLY OWNED Near Chaxhill, Westbury-on-Severn
• £12 per UDA
• 3 year tie in with a £20,000 Golden Hello
• 50% split on Private and Labs
• Hours 8.30am to 5.30pm
• Excellent private potential
• Existing, well maintained patient list
• 3 Fully computerised surgeries, Digital X-rays, Rotary Endo
• Hygienist and full support team
• On site Parking
• 20 minutes from Gloucester. 30 minutes from Cirencester. 30 minutes from Cheltenham
• Available now, start date negotiable

Compliance Requirements

• Full NHS Performer Number
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
