Vacancy Introduction
An exciting opportunity has arisen for an experienced and highly motivated Band 7 Clinical Lead Nurse to join our expanding community services across Leeds. In partnership with Leeds City Council and the GP Confederation, we are mobilising a new model of care delivering short-term intermediate care beds across four sites. This is a pivotal leadership role at the forefront of transforming services, improving patient outcomes, and supporting care closer to home.
We are seeking a dynamic and forward‑thinking nurse with a strong background in community or intermediate care, and a passion for delivering high‑quality, person‑centred care. You will provide clinical leadership across all bed bases, ensuring safe, effective, and compassionate care for patients. The role requires strong collaboration with multidisciplinary teams and system partners, alongside a focus on service improvement and innovation.
Key Responsibilities
- Provide expert clinical leadership across all bed bases, ensuring safe, effective, high‑quality patient care
- Lead and support multi‑disciplinary team (MDT) working, promoting a strong Home First approach
- Drive service improvement and innovation, supporting delivery of key performance indicators (KPIs), including reduced length of stay and timely discharge
- Ensure clinical governance, safety, and quality standards are consistently maintained
- Support staff through supervision, development, and leadership, fostering a positive and inclusive team culture
- Work collaboratively with system partners including Leeds City Council, GPs, and wider community services
Person Specification
- A registered nurse (NMC) with significant experience in community or intermediate care
- A confident and effective leader, able to influence, motivate, and develop teams
- Passionate about integrated working and delivering care closer to home
- Skilled in clinical decision‑making, risk management, and service improvement
- Highly organised, proactive, and able to manage competing priorities in a fast‑paced environment
- Committed to maintaining high standards of patient safety, quality, and experience
Additional Requirements
Full UK driving licence and access to a car is essential due to travel across multiple sites in Leeds.
