Job overview
The post holder will work as part of a multi‑professional team managing a caseload of patients in the community, ensuring the needs of the patients are identified and met. Promoting the independence of patients, assisting them to maintain or improve their own physical, psychological and social wellbeing. In addition to this the post will arrange timely responses overnight for urgent unplanned visits.
Main duties of the job
- Community Team Sister/Charge Nurse Band 6
- Access to a vehicle
- Work directly with patients, their families and multidisciplinary services to plan and facilitate the delivery of holistic, evidence‑based care
- Deliver care that supports patients to remain independent and avoid unnecessary hospital admission
- Co‑ordinate with the Senior Sister/Charge Nurse to provide clinical leadership and direct line management of the team members
- Contribute to service improvement by creating innovative solutions that enhance the patient experience
Person specification
Education / Qualifications
- Qualified RGN with current registration with NMC
- Practice assessor / Practice Supervisor
Experience
- Multidisciplinary team work
- Experience in managing the needs of complex patients
- Experience of conflict management
- Care Planning
- Experience working in a community setting
- Experience supporting students
Skills / Abilities / Knowledge
- Ability to work unsupervised or with minimal supervision
- Ability to work under pressure
- Insight into current issues relating to delivery of community/primary care
Other requirements
- Car owner/driver insured for business use
