What you will do

  • Facilitate safe and timely hospital discharges by assessing patients’ social care needs and devising comprehensive discharge plans.
  • Collaborate with multidisciplinary teams, including medical staff, families, and external agencies, to ensure cohesive care planning.
  • Conduct assessments and reviews, ensuring compliance with relevant guidelines and frameworks.
  • Provide emotional and practical support to patients and their families, navigating through challenging transitions.
  • Maintain accurate and detailed documentation in line with organisational policies and procedures.

Requirements

  • Two years of post-qualified experience.
  • No permanent social worker role for the past 12 months.
  • One-month notice period required.
  • Salary £28.40 per hour through umbrella company, inclusive of HPIL.
