What you will do
- Facilitate safe and timely hospital discharges by assessing patients’ social care needs and devising comprehensive discharge plans.
- Collaborate with multidisciplinary teams, including medical staff, families, and external agencies, to ensure cohesive care planning.
- Conduct assessments and reviews, ensuring compliance with relevant guidelines and frameworks.
- Provide emotional and practical support to patients and their families, navigating through challenging transitions.
- Maintain accurate and detailed documentation in line with organisational policies and procedures.
Requirements
- Two years of post-qualified experience.
- No permanent social worker role for the past 12 months.
- One-month notice period required.
