Community Psychiatric Nurse

Role Overview

Full time Band 6 Community Psychiatric Nurse (CPN) for the Locality Mental Health Team supporting the North area of Lincoln.

Responsibilities

• Provide high quality person‑centred care to patients, families, and carers within a Multi‑Disciplinary Team.
• Act as Lead Healthcare Professional, undertaking comprehensive evidence‑based assessments and supporting psycho‑social interventions to help service users work towards recovery goals, build self‑resilience, and promote independent living.
• Engage with Community Mental Health Nurses, Recovery Practitioners, Mental Health Wellbeing Practitioners, Psychological Intervention Facilitators, Nursing Associates, Community Support Workers, Psychiatrists, Psychologists, Admin staff and partner organisations including Neighbourhood Teams, Integrated Placement Based Teams, Adult Social Care, GP surgeries, and other partners to ensure best outcomes for patients and carers.
• Deliver services as a mobile clinician, travelling independently within the community and employing a hybrid approach to working.
• Participate in service development, contribute to organisational improvement and adapt to changes within a complex and evolving service.
• Receive robust clinical and managerial supervision, and engage in training to maintain up‑to‑date evidence‑based practice.

Qualifications and Skills

• Band 6 nursing qualification and qualified as a Community Psychiatric Nurse.
• Strong commitment to a recovery‑focused way of working, with the ability to empower and instil hope in service users.
• Excellent communication, leadership and teamwork skills.
• Flexible, proactive, and able to work under pressure and adapt to change at short notice.
• Demonstrated enthusiasm for service development and ongoing professional development.
