Position Overview
An exciting opportunity has become available to join the Short-Term Services Team. There are two full‑time positions and one part‑time role (0.8 FTE, 29.5 hours per week). These vacancies are open to both progressed and non‑progressed Social Workers.
Responsibilities
- Conduct adult needs assessments in line with the Care Act (2014).
- Manage and respond to safeguarding concerns.
- Apply the Mental Capacity Act (2005) to assess capacity and make best‑interest decisions.
- Deliver person‑centred and evidence‑based practice.
- Manage a diverse and complex caseload, including proportionate assessments and safeguarding.
- Collaborate with multidisciplinary teams, hospital therapists, and wider Adult Social Care Teams.
- Support patient flow through discharge pathways and assist with safeguarding risk identification.
Qualifications and Skills
- Strong understanding of adult social care frameworks.
- Excellent communication and interpersonal skills.
- Ability to work autonomously within policies and professional standards.
- Effective time management and organisational skills for caseload management.
- Proactive, self‑motivated approach.
