About The Role
Join a dynamic and supportive therapy‑led team and embark on a fulfilling journey as a Community Reablement Assistant. In this flexible and rewarding role you will work alongside Occupational Therapists and Reablement Officers, playing a vital part in helping individuals regain their confidence and independence after a hospital stay. As the first point of contact for those returning home, you’ll provide essential support and bring positivity to their recovery journey, making a meaningful difference in their lives every day.
What you’ll do
- Supporting people with their personal care and empowering them to do as much as possible on their own.
- Monitoring and providing feedback on the progress of people’s goals.
- Assisting with medication and encouraging independence.
- Completing care records and other relevant paperwork.
- Facilitating access to community groups and services.
A valid licence and use of a car for work purposes will be required as the role involves visiting people across the Bournemouth, Christchurch and Poole area. A valid licence to drive in the UK is required as the role involves travelling locally and lone working.
35 hours per week. Working 5/7 days including alternate weekends.
