Job Overview

Our teams support patients to live independently and improve their quality of life in the places they call home. As a Community Support Worker, you will work closely with a multidisciplinary team of clinicians – such as physiotherapists and nurses – to provide ongoing clinical activities, rehabilitation, recovery, nursing tasks and support with daily routines and personal care.

Responsibilities

  • Undertake a range of delegated clinical, therapy and nursing-led activities to support service users on the Integrated Network Team caseload.
  • Provide safe, patient‑centred and effective clinical activities in the community setting.
  • Work within adults’ services to deliver a full range of healthcare activities.
  • Collaborate flexibly with other teams or services, both within the locality and Sirona wide, when short‑notice needs arise.
  • Be able and willing to travel throughout the service delivery geographical area.

Qualifications and Experience

  • Working knowledge of clinical, therapy and nursing procedures relevant to community support.
  • We are also interested to hear from occupational therapy or physiotherapy students nearing qualification who are looking to build confidence and gain valuable experience within community healthcare.
  • Strong communication skills, empathy and a commitment to patient care.
