Job Description:

The purpose of the DischargeFlowCoordinator role is to ensure that patients have a seamless journey on discharge and that the patient’s pathway is effectively managed across site. The primary duty is to facilitate safe and timely discharges, ensuring patient do not stay longer than necessary in acute hospital bed, working closely with doctors, nurses, social workers and therapist to coordinate care packages, arranging patient transport, home equipment and referrals to external care providers, acting as the primary point of contact for families and escalating any delayed on transfer of care.

We are seeking an energetic, dynamic, and enthusiastic individual with strong organisational skills for the role of DischargeFlowCoordinator.

Epsom and St Helier University Hospitals NHS Trust provides a range of medical services to approximately 490,000 people living across southwest London and northeast Surrey. In addition, we provide more specialist services, in particular renal and neonatal intensive care, to a wider area, covering parts of Sussex and Hampshire.
