You will work as part of a small multidisciplinary team that provides mental health services to adults. The role involves delivering practical and emotional support, assessing needs, and providing social care services. You will collaborate with the Flexible Assertive Community Treatment (FACT) Clinical Lead and other senior staff to ensure coordinated and effective care. Responsibilities include working within the Care Programme Approach (CPA) and Care Management processes, maintaining a strong focus on recovery principles, and supporting service users to achieve their individual goals and improve their wellbeing. You will help implement recovery-focused care plans for an allocated caseload, ensuring engagement and regular access to appropriate services. A key aspect of the role is promoting independent living, empowering service users, and contributing to strong therapeutic relationships that are attentive, sensitive, compassionate, and honest. Additionally, you will assist in developing practical support plans for service users and their carers, preserving dignity, independence and quality of life.
Responsibilities
- Review information available to staff and service users for its quality and relevance.
- Identify early signs of relapse by monitoring service users’ progress, level of functioning and mental state and alert appropriate staff involved in the care.
- Report regularly to care coordinators and key workers.
- Maintain accurate records as required by existing procedures, entering appropriate details on case notes or electronic patient records.
- Ensure the service user understands and has a clear pathway of care across sector or agency boundaries with key contact points or named individuals.
- Participate in and be responsible for appropriate elements of staff development as part of the STaR Team meetings when required.
Qualifications
- NVQ Level 3 qualification or mental health certificate Level 3, or agreement to undertake such training as soon as resources permit.
- NVQ/Mental Health Certificate Level 2.
- Experience of working in a healthcare setting.
Desirable Criteria
- Venepuncture training.
- Basic written communication skills.
- Ability to create innovative solutions to empower service users.
- Ability to provide practical support with daily living activities.
- Three years experience of providing mental health services from within a team setting.
- Car Driver.
