The Role
£22,932 – £27,685, pay award pending. An exciting opportunity has arisen in Bolton to join the Transition team working with young people age 14‑25 with special education needs and disabilities. The successful candidate will work with both Children and Adult services to ensure a smooth transition for young people in preparing them for adulthood.
The social worker will be responsible for discharging duties in accordance with the Care Act 2014, undertaking holistic assessments, and developing transition plans that will inform the appropriate future pathway and support for the young person. The role also involves identifying and completing assessments of informal carers.

About You

Ability to communicate, develop, maintain and improve relationships while working in collaboration with young people, their families, and key partners.
Work with commissioners alongside the Deputy/Team manager to develop the market and ensure sufficient, effective, and high‑quality provision in the local area.
