Job Summary

Midlands Partnership NHS Foundation Trust is seeking a Rapid Discharge Facilitator to support the North Integrated Discharge Hub at Royal Stoke Hospital. The role operates from the triage hub providing hospital discharge guidance and support to referring acute hospital trusts. The candidate will work autonomously within the hub‑and‑spoke model, using professional judgement and initiative while collaborating with multidisciplinary teams.

Responsibilities

  • Triage referrals from inpatient wards for patients with complex needs requiring ongoing support post‑discharge.
  • Assess patients’ ability and potential for reablement and determine discharge pathways.
  • Coordinate admission‑avoidance assessments to prevent hospital readmission.
  • Provide accurate guidance to service users, families, caregivers, and professionals.
  • Work independently within the hub‑and‑spoke model, using professional judgment and initiative.
  • Collaborate with clinical and professional networks across community teams.

Qualifications and Experience

  • oLevel 3 Diploma in Adult Care (Regulated Qualifications Framework) or equivalent, with additional knowledge of care practices to diploma level.
  • Experience working in a team, fostering good interpersonal relationships and contributing to individual needs development.
  • Management of complex discharges and procurement of care packages.

Desirable Qualifications

  • oLevel 4 Diploma in Adult Care (Regulated Qualifications Framework) or equivalent.
  • Experience working within an integrated team.

Employment Details

Band: 4Salary: £28,392 to £31,157 per annumContract: PermanentWorking Pattern: Full‑time or part‑time (pro‑rata)
