Consultant Geriatrician

CDDFT has developed an integrated, consultant‑led frailty‑care model that includes the Frailty SDEC Unit, the H@H service, and the Geriatrician of the Day initiative. These services work together to improve outcomes for patients living with frailty, reduce unnecessary hospital admissions, and support care closer to home.

Job Responsibilities

  • Lead assessment and management of patients living with frailty, including those with complex needs and frailty syndromes (e.g. falls, immobility, delirium).
  • Make treatment decisions and provide ongoing care for patients, inclusive of those in the H@H service.
  • Support the Frailty SDEC / H@H pathway by providing timely, specialist‑led input.
  • Act as the Geriatrician of the Day to ensure rapid Comprehensive Geriatric Assessment and appropriate streaming of patients to the right care setting.
  • Facilitate early discharge planning and step‑down care from inpatient wards to home‑based services.
  • Participate in clinical governance activities, including maintaining up‑to‑date protocols, guidelines, and contributing to clinical audits.
  • Revise and develop evidence‑based protocols for frailty care and undertake associated administrative duties; support innovation in service delivery aligned with national strategies, including the NHS 10‑Year Plan.
  • Maintain continuing professional development in line with RCP guidance.
  • Act as an educational and clinical supervisor for junior doctors and support training of junior doctors and multidisciplinary team members across inpatient and outpatient settings.
  • Participate in a 7‑day rota and on‑call service in collaboration with the Frailty SDEC and other clinical teams.

Person Specification

  • Full registration with GMC and inclusion on specialist register (or pending in next 6 months).
  • Evidence of continuous development in the specialist field of practice.
  • Strong clinical skills in acute medical patients and geriatric medicine.
  • Experience working within a multidisciplinary team with effective communication skills.
  • Experience supervising and teaching trainees, including evidence of career planning through appraisal.
  • Up‑to‑date knowledge of professional development and clinical efficacy work.
  • Experience liaising with community services and contributing to service development.
  • Support for innovation in frailty‑care delivery and alignment with national strategies.
  • Eligibility to undertake a Disclosure and Barring Service check.

Salary: £113,565 to £150,569 per year (YC72 Pay Scale, pro rata)
