About the Candidate
We are looking for an experienced, confident, and motivated practitioner who is passionate about safeguarding adults, promoting independence and improving outcomes for vulnerable people. You will need a sound knowledge and experience of CHC and a comprehensive understanding of the National Framework for NHS Continuing Healthcare and NHS‑Funded Nursing Care 2022. You will also bring a constructive, forward‑thinking approach—embracing change, promoting high standards and helping shape a positive and inclusive team culture. This is an exciting opportunity to join Wirral Council as an Experienced Social Worker (Adults)—a pivotal role within our Adult Social Care service, championing high‑quality practice across safeguarding, adult protection and complex casework.
Responsibilities
- Manage a case load where circumstances and needs have indicated high levels of risk and complexity.
- Ensure that individuals and carers are involved in timely assessment support planning and review processes, and that care and support plans are person centred and focussed on individual outcomes.
- Ensure that effective risk management, decision making, and preventative work is undertaken to promote independence of individuals and carers whilst taking into account the need to safeguard people effectively.
- Ensure that complex cases that involve partners and their respective legal, statutory, and regulatory functions are referred to the Safeguarding Unit for wider consideration.
- Take a constructive approach towards innovation and changes in methods of working which best improve the outcomes for individuals.
- Multi‑disciplinary working with health professionals, identifying services & support and utilising funding streams in the locality.
- Supporting casework for existing funded clients, including those approaching review.
- Creating and monitoring data, with supporting workflow and system developments, to manage applications and oversee cases effectively.
- Work closely with Legal Services to support correspondence relating to funding applications and decisions.
- Work closely with providers across all sectors on best practice in case recording, case risk assessments, positive behaviour management, and the collation of evidence to support applications.
- Completed strength based holistic assessments. Undertake Care Act Assessments with a person who could be in supported living, a care home setting, or at home in the community with a Package of care.
- Hold a case load of people who could be eligible for Continuing Healthcare (CHC) and complete relevant documentation as required.
- Complete Mental Capacity Assessments, B.I. when appropriate.
- Effective time management, plan and coordinate case load, ensure all documentation is completed in a timely and effective manner.
- Monitoring Service provision and Standards in accordance with Registration and Inspection Department.
- Complete CHC Checklists. Attend DST/MDTs with Health Colleagues.
- Complete Community Reviews /Placement reviews-ensuring that needs are being met in the appropriate environments. Promoting Independence and Empower.
Requirements
This post requires a DBS check. Eligibility to work in the UK will be verified in the final stages of the selection process. Candidates must have a right to work in the UK.
