Main duties of the job
The role will include supporting teams and patients in a variety of care environments – primarily community hospitals but also virtual wards and patient’s own homes. Always ensuring that the best is obtained from medicines.
- Improve near-patient services by managing medication on the ward, including stock/assessing patient’s own medicines.
- Electronic ordering of medicines.
- Confirming medication histories with patients and reconciling medicines.
- Counselling patients about new medicines.
- Liaising with clinical pharmacists, multidisciplinary team (MDT) members and primary care colleagues.
- SOPs and safe systems development and provision.
- Facilitate the effective discharge of patients and their medicines. This may include supporting patients and care staff post discharge.
Qualifications and / or Knowledge
- Current registration with the GPHC
- Accredited Checking Technician qualification held, or relevant experience Medicines Management accredited
Experience
- Experience in Specialist Pharmacy Technician ward-based or Medicines Management role
- Experience of POD’s scheme
- Experience of working with electronic Pharmacy systems
Aptitude and Abilities
- Ability to train and convey knowledge to others, where information may be sensitive
- Ability to communicate complex information and use appropriate questioning techniques and documentation methods to ensure understanding
- Able to make judgements on immediate priorities for work
- Able to analyse usage patterns, activity data to plan ward stock etc
- Able to work on own initiative as well as part of team
- Knowledge of relevant Pharmacy/Medicines legislation
- Some ability to speak, read and/or write Welsh, or an eagerness to learn.
- Ability to plan rotas, supervise junior support staff
