Senior Social Worker – Transition & Empowerment

West Sussex County Council is seeking a qualified Social Worker to join our Transition Teams in Horsham. This permanent, full-time role focuses on supporting young people aged 16 with lifelong disabilities as they transition to adulthood. You will conduct assessments, manage a varied caseload, and collaborate with partners to improve outcomes.

The ideal candidate will have a degree in social work and relevant registration, along with strong communication and assessment skills. A valid driving license is required.
