Good Afternoon, I am currently representing Enfield Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience.

Specialist SEN & Disability Outreach Worker

Location: EN1 3ES

Responsibilities

  • To work as a specialist SEN and Disability outreach worker, within Enfield’s Service for Disabled Children.
  • To build effective partnerships with parents/carers, professionals, and service providers to ensure positive outcomes.
  • To support children and young people and their families to access effective and person‑centred short breaks and community activities.
  • To provide advice, information and guidance to children and young people with SEN and disabilities and their families.
  • To undertake short break assessments and reviews to ensure that the right support is provided and continues to meet identified needs.
  • To work alongside the SEND Transition Manager in ensuring that transition pathways are progressed in a timely way and to contribute to Moving On events.
  • To ensure that the participation and views of children and young people with SEN and Disabilities inform service development.
  • To undertake the lead professional role for children and young people with SEND and disabilities when appropriate.
  • To work alongside Social Workers to ensure that the support plan is progressed.
