Short Term Services Support Worker

Our short‑term services are recruiting for a Support Worker to join our team on a part‑time (30hrs) permanent contract. You will provide care and support to people in the community with a focus on maximising independence and delivering a high standard of care to a variety of people, including end‑of‑life care.

Following a period of acute ill health or hospitalisation, you will undertake short‑term care as defined in care & short‑term service plans or delegated tasks from Therapists/Nurses, contributing to the assessment process and ongoing monitoring and review of people’s goals and reporting any changes to a person’s medical condition.

This role is part of an exciting development of services to help prevent avoidable hospital admissions, working alongside intermediate care teams and other community teams.

You will work as part of a team, including lone working within the community, across Torbay and South Devon to provide integrated health and social care, working closely with other organisations.

Shift patterns are flexible, covering 0800 hrs to 2200 hrs, 7 days a week. Each shift is paid in full, regardless of direct contact time, and travel time is paid at your full hourly rate with a generous mileage allowance. Shift enhancements are available for evenings, weekends, and bank holidays.

Responsibilities

  • Assist and promote independent social and living skills, including mobility, personal care, bathing, dressing, continence, nutrition, hydration, prompting, and medication administration where required.
  • Use therapeutic moving and handling skills, often with prolonged physical effort, and employ a wide range of moving & handling equipment.
  • Undertake specific clinical tasks as delegated after relevant training, including baseline observations, simple wound care, and dressing management; recognise deteriorating health and use observations to assist in assessment and escalation through early warning score and SBAR.
  • Follow goal plans and use observational skills to evaluate and modify interventions to participate in the re‑ablement of people, engaging them as an active part of the process.
  • Fit and adjust community equipment to people and their environment, teaching correct use of specialist equipment.
  • Provide feedback to team or clinical leads on outcomes, concerns, progress, and deterioration relating to goal achievement.
  • Form professional relationships with people and communicate effectively with them, team members, families, carers, and others, managing any barriers to effective communication.
  • Ensure patients, relatives, carers, and professionals are kept fully informed of progress following intervention, communicating complex information to meet varying levels of understanding and overcoming barriers such as hearing loss, cognitive impairment, dysphasia, pain, fear, and cultural differences.
  • Contribute to ensuring a safe working environment for self and others, reporting potential risks in line with local policies and procedures.
  • Only undertake work within own competence and seek advice or supervision from Team or Clinical Lead as necessary to ensure appropriate patient management and care.
  • Act on local health and safety policies, promptly recording any accidents, incidents, or other untoward events involving self, patients, or staff.
  • Observe and evaluate service users’ response to the care plan, communicating changes in need or circumstances to the appropriate member of the service in a timely manner.
  • Provide reports on service users using the service IT system, while complying with Trust Policies and Procedures.
  • Undertake training relevant to the post and apply it in practice, supporting other teams during periods of reduced work.

Qualifications and Training

  • Experience of personal care and/or clinical procedures, to NVQ2 level qualification or equivalent knowledge.
  • Basic IT skills – Microsoft Word, Outlook, Excel.
  • NVQ3, QCF or Diploma Level 3 in Health & Social Care.
  • Basic Food Hygiene Certificate.
  • Good literacy, numeracy, verbal and written skills, including record‑keeping skills.
  • Excellent interpersonal and communication skills, with ability to communicate with individuals of varying levels of understanding.
  • Knowledge and understanding of equipment used to promote independence.
  • Ability to carry out safe practice when moving and handling users and equipment.
  • Understanding of relevant health and safety legislation for moving and handling.
  • Ability to work flexibly, including unsocial hours and shift patterns such as weekends and bank holidays.
  • Full UK driving licence and access to a vehicle.
  • Subject to a Disclosure and Barring Service check.
  • Please note: This role is not eligible for sponsorship under the UK Home Office Skilled Worker visa route, due to the increased salary threshold introduced in early May 2025. Therefore, we are only able to consider applications from individuals who already have the right to work in the UK.

  • Ability to travel efficiently around the Bay is important; you should have a full driving licence and vehicle.

Contract and Salary

Permanent contract. Salary: £25,760 to £27,476 per annum pro rata (based on 30 hrs per week).
